    For a complete panel and or targeted levels you will need to go to an online specialist lab as

    SUPER MOD
    Frank Lewis has suggested. In my experience Health Insurances will not pay for preventative maintenance lab tests. Especially Hormone levels. The blood tests prescribed by your primary care doctor are only standard panels.

    Hey bro, Primary care could request BW per your request.
    Or
    Stop at Quest Diagnostic lab and request blood work.

    Please note certain laboratories or Dr office may require Prescription

    It all depends on your state policy.
