For a complete panel and or targeted levels you will need to go to an online specialist lab as Frank Lewis has suggested. In my experience Health Insurances will not pay for preventative maintenance lab tests. Especially Hormone levels. The blood tests prescribed by your primary care doctor are only standard panels.

Stop at Quest Diagnostic lab and request blood work.
Please note certain laboratories or Dr office may require Prescription
It all depends on your state policy.
